With the great majority of queens the answer is no but it does happen on occasion. If you have a queen who has made her way above there is more often an alternate explanation.

Jim is correct. A young queen that has not started laying has been known to slip thru on occasion. Ussually the culprit is damage to the wires. Hold AT An angle and look for the bent wire.

Sometimes.

Yes. Sometimes they have raised a queen above the excluder because you gave them some brood on both sides or they superseded and the queen came back to the top instead of the bottom.
