one of my splits has resulted in a small queen - it was just slightly bigger than the workers and far smaller than the other queens of the same age.
after 2 weeks from hatching it was still in the hive and not laying eggs - then i checked again on the 3rd week and seems she is gone completely, i checked once again few days later and didnt see it.... so i gave them a frame of eggs and now they are making a new queen - that also indicates that the queen is not longer there

was the queen naturally eliminated because it was small or this was a mere coincidence and perhaps was lost or killed during the mating flight ?

The queens need to be well fed in order to survive the mating flight. She probably got lost or killed during the flight. You may want to add some nurse bees to your split to ensure the queen is well fed. I assume you have the nutrition part (flow or pollen patty/syrup) covered.
