As has been pointed out to me by a number of people, all too often when the queen that comes with the package is dead on arrival it means the bees killed her because there's already one in package. Not saying this is always the case, but it happens. You should be aware of this possibility and be observant when you install the new queen to see how the bees are responding to her. If the new queen is killed, it's almost certain you have a queen in the hive already.
Update: I found my replacement package queen today, released and alive and well. So much for worrying
