In September (2020) I treated my hive with one strip of Formic Pro placed between the 2 brood chambers. I had left 2 supers of honey on the hive during treatment with the intent on leaving those honey filled supers for the bees. This week I discovered the hive had died. Is it safe to now take the honey from those supers for my own consumption? Thank you.

Formic Pro is approved for use with the honey supers on, so even the folks that are going to quote 'use according to label', self included in that group, will say yes, you can extract and sell honey taken with formic pro on.

It may not taste right-check it after you harvest including moisture. If it taste funky, feed it back to the bees.
