The second deep on my hive is just about full. Should I add another deep, or should I add a medium super and a queen excluder. I am not interested in taking honey from the bees this year, it is my first year with them.

Indications are you have only one hive? You have choices... add a deep, and when you split next spring you'll have three deeps of drawn comb hopefully, which will help your expansion go well.
Add a medium or shallow and get some honey this year, although it may be too late. Add the medium anyway, and any comb they draw and honey they store they can use for winter feed, and it will help them get an early start next year. I wouldn't use a queen excluder, as the honey in the top deep will function as an excluder. Plus you don't want the excluder on in winter time in case the cluster needs to move up into that top box.
Plan now for split/swarm control next spring.
