I have several bait hives that did not catch a swarm last year---Does the foundation have to be changed from year to year, or only to add scents to them...The foundation is not torn or anything---just whitened with age.

Foundation does not "go bad", so to speak, but foundation is also not of much use in attracting swarms. You can add scents (lemongrass oil, nasanov), but it will also help if you can replace at yeast one of your frames of foundation with old, dark drawn comb (or any drawn comb if you can't find the old dark variety).
I spray the comb in my swarm traps with a liberal dose of BT before hanging. Seems to help limiting wax moth damage in between checks.
