What is the foam on top of extracted honey that has sat for many days?
I know some of it is particulate matter floating. Is it wax and small air bubbles?
Any tips/techniques on getting rid of the foam on top of extracted honey?

That's exactly what it is... skim it off and discard (in your belly on biscuits is a good way,) or stir it back up.
One Thing I have done that works is after the foam finishes floating to the top. Take a paper towel and gently place on top of the foam and honey. Gently pat it down to make sure it makes contact across the entire surface. Then quickly pick the paper towel up by it's four corners. And remove (over a surface that can be cleaned off - drips). Either discard the paper towel or set it out where your bees can collect up any honey on the towel.
I skim it put it in it's own jar and use it for my toast does not run and tastes just fine.
