    You should never add an acid to syrup with heat. An 7 determined amount of HMF will form, and can be a problem. Yes..too much vinegar, but that isn't the real problem.

    Your observations are good....but some more info would help narrow things down....specifically what about open brood.

    EFB would tend to show with a solid pattern of young larvae, and more spotty...

    The sugar is fed to the bees and stored in the empty cells the bees were planning to overwinter in.
    The empty box is at the bottom, and is to give the bees a place to cluster.
