Can you combine the bees from several different hives to make one? Is this the way packages of bees are made up?

Kind of... You gotta really confuse em with lots of hive smells or they'll fight. Soo just enough to make one hive, I'd keep em separate for a day or two till the queen smell has dissipated.

Yes.

Yes.
In this case confusion is your friend. If you get some frames of bees from three or more hives they usually don't fight.
