lost hive

While we had some warm weather, I went into my two hives and found one almost completely dead. From what I was able to research, the bees might have starved to death (buried head first into the comb). What suprises me is that there were two full supers and a gallon hive top feeder with 2/1 syrup on it. Anyone have any thoughts what happened?

Queen was missing. Could not find in dead pile of bees.

Was there an empty super between the honey and feed? I have lost a few hives this way. The bees didn't realize that there was food above the empty.

No empty super. Both were full.
